Relative blood volume profiles hours after loop diuretic administration: a systematic review and meta-analysis

Background and AimsPlasma refill rates can be estimated by combining measurements of urine output with relative blood volume profiles. Change in plasma could guide decongestive loop diuretic therapy acute heart failure.ObjectivesTo assess average profiles generated from two or three follow-up obtained hours after administration subjects without baseline congestion.MethodsA systematic review English-, French-, Spanish-, German-language articles using MEDLINE (1964 to 2019), Cochrane Reviews (1996 EMBASE (1974 2019). Search terms included diuretics, hemoconcentration, volume, volume. We studies adults given a at least one measurement. A single author extracted subject- group-level measurements, aggregated them when needed, converted changes.ResultsAcross all 16 that met the pre-specified inclusion criteria, maximally decreased 9.2% (6.6 12.0%) returned 3 more hours. When compared against congestion, those congestion experienced smaller decreases across periods (p-value 0.001) within final period.ConclusionsSingle doses diuretics produce measurable changes follow distinct for congestion. measured alongside output, these may used estimate — potential patient-specific targets serial doses.
